If you need a HIPAA AI gateway, the number that matters is how many providers and models you can actually send PHI to. Aptible's LLM Gateway covers Anthropic, OpenAI, and Bedrock-hosted models under its BAA. Concentrate covers seven BAA-eligible providers (OpenAI, Anthropic, Google Vertex AI, Azure, Fireworks AI, Fireworks on Azure, and Baseten) and 71 BAA-eligible models, with more providers coming.
Aptible's pricing page says its gateway "covers Anthropic, OpenAI, and Bedrock-hosted models under a single BAA." Its supported models docs describe three routes: Anthropic, OpenAI, or Amazon Bedrock. Google is not one of them, so Gemini is not on the list.
Today, BAA-eligible providers on Concentrate include:
| Provider | What it adds |
|---|---|
| OpenAI | GPT-6.1, GPT-6 and the GPT 5.x family |
| Anthropic | Claude Opus 5.5, Sonnet 5.5, Haiku 5.5 and Fable |
| Google Vertex AI | Gemini 4 Argon, Gemini 3.8 Flash and other Gemini models |
| Azure | Frontier models on Microsoft's cloud |
| Fireworks AI | Open-weight models |
| Fireworks on Azure | Open-weight models |
| Baseten | Open-weight models |
As of October 2026, that adds up to 71 BAA-eligible models. See Model Fortress for the live list. We keep adding BAA providers, and one BAA with Concentrate covers each new one as it's added. No new contract, no new integration.
More providers means more model choices and more places to fail over.
Yes. The same API reaches 207 models across 31 providers. Use the BAA-eligible ones for PHI and everything else for the rest of your product, with one key, one bill, and one set of controls: provider allowlisting, data redaction before a request leaves the gateway, SSO, and role-based access control.
Concentrate is free to use. BAA access is a $499/month subscription, with no per-request fees and no markup on tokens. Aptible bills LLM Gateway usage at list model pricing plus a 15% markup, so its cost grows with every token you send. On Concentrate, the subscription stays the same whether you send a million tokens or a billion.
Under HHS guidance, a cloud or AI service that handles ePHI for you must sign a BAA. You still own your own HIPAA obligations. For more, see our HIPAA Compliant AI Gateway overview.
Does Aptible's LLM Gateway support Gemini? Aptible lists Anthropic, OpenAI, and Bedrock-hosted models. On Concentrate, Gemini is BAA-eligible through Google Vertex AI.
Can I use open-weight models with PHI? Yes, on BAA-eligible providers like Fireworks AI, Fireworks on Azure and Baseten, with a signed BAA and a HIPAA-enabled organization.
Is any model HIPAA compliant on its own? No. A model can be used with PHI under a signed BAA; compliance depends on how it's deployed.
Does Concentrate sign a BAA? Yes, with a $499/month subscription. The gateway itself is free to use, with no per-request fees or token markup. See concentrate.ai/compliance.
Does one BAA cover every BAA-eligible model? Yes. Every model on a BAA-eligible provider, including providers added later.
